SMPCP Swan Hill Health and Wellbeing Partnership

The Southern Mallee PCP Swan Hill Health and Wellbeing Partnership's purpose is to support collaboration between Swan Hill agencies relating to health and wellbeing of the local communities, and to provide a forum for Swan Hill agencies to plan, implement and evaluate the Swan Hill Public Health and Wellbeing Plan (Partnership Action Plan) as part of the Southern Mallee PCP Strategic Plan 2017-2021 and linkages to the Swan Hill District Health - Health Promotion Plan.

Membership is open to local Swan Hill agencies. Current members are:

Initiatives

Victoria Against Violence Campaign 2017

The SMPCP Swan Hill Health and Wellbeing Partnership was successful in obtaining funding from Women's Health Loddon Mallee to hold a Swan Hill Put Your Hand Up morning tea at Cafe 202 and produce a video on why preventing family violence is important in Swan Hill.

SMPCP Swan Hill Aboriginal Mental Health Partnership

This Partnership (orginially a working group of the SMPCP Mental Health & Wellbeing Committee) formed due to the interest shown by Bendigo Health and Mallee District Aboriginal Services to support one another to better coordinate care for shared mental health clients, facilitate discharge from Mental Health Services to MDAS, and create better understanding of mental illness by education and discussion.

The current Chair is MDAS and the SMPCP staff facilitator is Eden Templeton.

SMPCP Swan Hill Community Services Network

This Network meets biannually to bring together professionals to share information and network, with a focus on disability and aged care services.

Better Together Community Engagement Framework 2019

The Better Together Community Engagement Framework 2019 provides information, a framework, process and resources to consider when planning, implementing and evaluating stakeholder/community engagement. The Framework is designed to be used by anyone wishing to engage communities of interest, including geographic location, sports, culture, special interest, organisations and stakeholders, and can be adapted to suit specific initiatives, as needed.
