Southern Mallee Primary Care Partnership 

The Southern Mallee Primary Care Partnership (SMPCP) is located in north-western Victoria. It is comprised of three Local Government Areas; Buloke Shire, Gannawarra Shire and Swan Hill Rural City (not including Robinvale). SMPCP is an established network of local health and human service organisations working together in an effective way to improve the health and wellbeing of the local Southern Mallee communities.

SMPCP Mental Health & Related Services (Eligibility & Entry Criteria) Resource Update

The Southern Mallee Primary Care Partnership Mental Health & Related Services (Eligibility & Entry Criteria) Resources have been updated! These resources are used to increase awareness of, and referral to, a range of Mental Health Services and supports, increase and improve communication between agencies, and provide agencies with a resource which clearly defines the Eligibility and Entry Criteria for Mental Health and Related Services across the Southern Mallee. Buloke Living Book Launch!

Loddon Gannawarra Health Needs Analysis

An analysis of health in the Gannawarra and Loddon Shires has identified four priority areas – diabetes, heart health, mental health and oral health. The Analysis was undertaken by the Loddon Gannawarra Health Services Executive Network, which is made up of 12 health and community organisations across the two shires, including SMPCP. SMPCP Strategic Planning 2017-2021

Analysis of the current SMPCP Plan achievements and planning for the next SMPCP Strategic Plan 2017-2021 has been underway. We held a Strategic Partnership Forum to bring together the Board of Management members with the broader partnership.
